Install:

• Upload ./inc/plugins/wnr.php into your ./inc/plugins/ folder
• Upload ./admin/modules/tools/wnr.php into your ./admin/modules/tools/ folder
• Go to your PluginManager and Activate the "Extra Registration Safety" Plugin.
  

Settings:

• Show Extra Check While New User Tries To Register?
• Whats The Error That Will Be Displayed If They Dont Fill In The Correct Answer?

* New Users Need To Answer A Question Correct Before Completing The Registration.

Update:
Deactivate 1.2.* version; Upload 1.4 version && activate.

If you enter more than one question will it ask them all or will it show 1 random question?

I'd like to make a suggestion for this plugin:

If for some reason a user is unable to answer the current question, an option to refresh the question for another one would be appreaciated.

Refresh the page, only the password field will be blank again.
And its you that enters the questions; don't ask difficult things.

LOL..yeah, mine are so basic, and I give them a choice of answers. Like "What color is the sky: blue or pink?" If someone can't answer that, I don't want them on my forum!
